A work in progress...

Jemez Aging in Place hosted a Community Needs Assessment to help our community identify the most important needs for our over 50 population to allow us to age in place in the Jemez. There are many interesting findings in the Needs Assessment, including that 98% of those who responded plan to stay in the Jemez area as they grow older. Thanks so much to New Mexico State University Department of Public Health Sciences for compiling the results and for their support in developing the Assessment report.

Even though our Needs Assessment has been compiled and published, your input still matters! If you haven't yet completed the Needs Assessment questionnaire, please download it below and send it to us by email, mail, or hand it to one of us.

The Needs Assessment questionnaire is for aging adults of the Jemez Valley and Mountains: let us know your needs and priorities to be able to age in place, to remain in your home and community for as long as possible, and to receive the care and support you need to do so.  Please give us your input by filling out this survey:

Click the above button to enable an automatic email pop-up or attach the completed Needs Assessment to an email message and address it to: jemezaip@gmail.com

Your responses will be confidential unless you otherwise note.
